Bill of Rights
You have the right to:
- access
needed health and social services
- receive
a fair and comprehensive assessment of your health, functional,
psychosocial and cognitive ability
- expect
you and your property to be treated with respect and dignity
in a way that protects your personal property and supports
a positive self-image
- be
free from mental, physical, sexual and verbal abuse, neglect
and exploitation
- privacy
and confidentiality
- participate
in the development of your plan of care and be consulted
if a change in your plan of care is needed
- refuse
all or any portion of the plan of care
- be
as pain free as medically possible
- know
who will be providing your care and what care they will
provide
- expect
your rights to be protected during research, investigation
and clinical trials if you are involved in a clinical research
project
- change
any or all service providers or caregivers at any time
- be
notified in writing of any denial, termination or change
in services or providers
- a
grievance procedure if you feel your rights are violated
or that you were treated improperly
- have
your wishes respected regarding your end-of-life decisions
- be
informed about the expected outcomes of care, treatment
and services, including unanticipated outcomes
- have
your disease process, progression, treatment plan and services
explained in a manner that's easy to understand
- participate
in all decisions that affect you directly regarding your
treatment plan, end-of-life decisions, or life activities,
friends and visitors
- choose
who will make health care decisions for you if you are unable
to make these decisions.
